The user undertakes to provide correct and complete information when filling out the form. The accuracy of the location data provided is essential to ensure that the service functions correctly.

NeuroCognitera Health merely acts as an intermediary between the user and the indicated clinic, and does not guarantee the quality of the service provided by the clinic. The choice and contact with the clinic are the sole responsibility of the user.
